Mental Health Transition Facilitator Team Lead

HopeLink Behavioral Health is seeking a highly skilled and compassionate Mental Health Transition Facilitator Team Lead to join our team. As a key member of our Youth and Family Services department, you will be responsible for managing the care of individuals on our caseload in the Transition to Independence Process (TIP) Program.
The TIP Program serves adolescents and young adults ages 14-29 with emotional and behavioral difficulties, providing a creative, wrap-around service model similar to an FEP or ACT model. As a Transition Facilitator Team Lead, you will teach skills to prepare individuals to live independently, obtain employment, advance education, or function more effectively in the community.
Key Responsibilities:
- Lead weekly team meetings and oversee case management for clients in the TIP Program.
- Provide individual, group, and family therapy, along with psychoeducational services.
- Conduct assessments for TIP, MHSS, and OP therapy, developing individualized rehabilitation/transition plans with clients.
- Facilitate discharge planning and coordinate after-care services for clients leaving hospitals.
- Act as a liaison with other care providers to ensure comprehensive and coordinated care.
- Deliver evidence-based psychotherapy to support clients in transitioning to community-based activities.
- Conduct risk assessments, provide crisis intervention, and offer dual diagnosis-focused services.
- Maintain accurate and detailed client records according to licensing and Medicaid standards.
Requirements:
- Master's degree in psychology, social work, rehabilitation counseling, or a related field with significant experience.
- Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW), or approved by DHP for licensure supervision.
- Child Welfare interest required, experience preferred.
- Bilingual English and Spanish preferred.
- Strong understanding of treatment approaches for emotionally and socially disturbed individuals, with knowledge of relevant psychiatric, psychological, and sociological concepts.
- Excellent written and presentation skills, with the ability to build rapport with clients, internal staff, and external providers.
- Ability to work independently with strong organizational skills.
- Experience with electronic health records (EHR) or a willingness to learn quickly.
Benefits:
- Monday - Friday 9:00 am - 5:00 pm with occasional evening No on Call or Weekends

- $70,000- 75,000 Estimated starting salary based on qualifications and related experience
