Gentiva Health Services is seeking a compassionate and skilled Palliative Care RN to join our team. As a Palliative Care RN, you will work with patients, their families, and other healthcare professionals to provide high-quality end-of-life care.
Key Responsibilities:- Conduct in-person and telephonic assessments to palliative care patients in collaboration with the interdisciplinary team.
- Provide education to patients and families on disease process, self-care techniques, end-of-life care, medications, nutrition, and dietary needs.
- Communicate any findings that identify appropriate preventive and rehabilitative skilled nursing or therapy needs to the PCP or AIM Palliative NP.
- Coordinate care with palliative team members and support hospice referral as appropriate.
- Document timely clinical records on all patients, including screening/assessment findings, physician orders, progress notes, and care plans.
- Nursing experience in palliative care, oncology, hospice, or geriatric care preferred.
- Additional experience in hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing is a plus.
