Palliative Care Nurse Practitioner or Physician Assistant Opportunity

Texas Health Physicians Group is seeking a dedicated and compassionate Palliative Care Nurse Practitioner or Physician Assistant to join our team. As a key member of our palliative care team, you will provide high-quality, patient-centered care to individuals and families dealing with serious illnesses.
Key Responsibilities:- Provide comprehensive palliative care services to patients and families in an inpatient setting
- Collaborate with healthcare teams to develop and implement individualized care plans
- Conduct counseling and education sessions to support patients and families
- Utilize electronic medical records to document patient information and communicate with healthcare teams
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Current NP or PA certification and Texas license required
- At least 1-2 years of palliative care experience
