Palliative Care Specialist Job Summary

At Community Healthcare of Texas, we are seeking a skilled Palliative Care Specialist to join our team. As a Palliative Care Specialist, you will work collaboratively with the Palliative Care Team and Texas Health Resources to provide high-quality Palliative Care services to patients and families within your scope of practice.

Key Responsibilities:
  • Review clinical information and coordinate patient care in consultation with the palliative medical physician, interdisciplinary team, and other healthcare practitioners.
  • Consult with patients' primary care physicians and other healthcare practitioners regarding patients' eligibility for palliative care, ongoing care needs, and medical management related to their serious/terminal condition.
  • Conduct in-person and telephone visits to assess/reassess patients' need for Palliative Care services and ongoing eligibility.
  • Manage health conditions and serious illnesses by developing individualized plans of care, reviewing medications and/or treatments, obtaining consultations or making referrals, and coordinating palliative services and/or hospice transitions.
  • Educate/counsel patients, families, and/or caregivers on preventative care, medical problems, psychological problems, symptom management, advanced care planning, and spiritual problems in conjunction with the interdisciplinary team to meet patients' total needs.
  • Participate in the Palliative Care IDG and complete required documentation to support Palliative Care eligibility and meet regulatory requirements for communication of findings to Community Healthcare of Texas.
  • Communicate with Senior Director, Medical Director, and appropriate hospital staff regarding any patient needs requiring medical intervention.


Requirements:
  • Licensed as an Advanced Practice Practitioner in the State of Texas.
  • At least 2 years of experience in geriatrics or palliative care.
  • Proficient skill using a computer is required; Microsoft office suite experience is required.
