Pharmacy Technician Medication Reconciliation Specialist

We are seeking a highly skilled Pharmacy Technician to join our team in Camden, NJ. As a Pharmacy Technician Medication Reconciliation II, you will play a critical role in ensuring the accuracy of patient medication lists and communicating with the healthcare team to ensure seamless care.
Key Responsibilities
Accurately validate prior-to-admission medication lists and take medication & allergy history from patients
Contact outpatient entities as needed for additional patient medication information
Communicate information to the healthcare team
Document/correct information in patient medication list in EPIC
Requirements
At least 4 years of hospital pharmacy technician experience required
Current NJ registration (or eligible upon hire)
Able to complete basic pharmaceutical calculations
