Clinical Leadership and Compliance

As a Home Health Clinical Manager at AccentCare, Inc., you will oversee the day-to-day clinical and office operations, ensuring services are in compliance with professional standards, agency policy and procedure, and state and federal regulatory requirements. You will serve as a back-up support for positions as needed, including PCM, CSS, and Leader in absence of the Territory - Administrator.
Clinical Oversight of Patient Care
================================
You will coordinate referrals, coordinate patient care with an interdisciplinary team approach, and assure the development, implementation, and updating of the individualized plan of care. You will evaluate the quality of clinical care, ensuring that patient needs are continually assessed, and collaborate with clinicians and physicians regarding patient transfers or discharges. You will review Discharge assessment data, Edit/Lock Discharge OASIS, and discipline only discharge. You will daily manage KPIs, workflow, escalation/complaint calls, and guide professional clinicians on implementing Emergency Care Protocols.
Clinical Oversight of Personnel
============================
You will assure clinicians, including contract staff, have required training, competency, and supervision. You will make patient and personnel assignments, ensuring that assigned staff have demonstrated competence in the required tasks/services. You will provide ongoing coaching and mentoring of field clinicians.
Quality and Compliance
====================
You will be the QAPI owner and participate in QA and performance improvement activities. You will review/investigate incidents, analyze for trends, and implement corrective action plans as needed. You will manage HHCAHPS distribution in SHP and Emergency Preparedness Planning, including Activation of EPP, Post Activation Reporting & Ongoing Event Monitoring and Reporting.
Requirements
============
* Registered nurse with current licensure to practice nursing in the practicing state
* Possess and maintain CPR certification
* Previous experience in home care setting with two years management or supervisory experience, preferred
* Proven ability to work within an interdisciplinary setting
* Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and is in good working order
* Prior use of Electronic Medical Records (EMR) preferred
