We are seeking an experienced Youth Health Navigator to join our Children's Home Health Care Management team in Albany, NY. The ideal candidate will possess strong communication and organizational skills, with the ability to build trusting relationships with families and provide comprehensive health home care coordination services for children with low to high acuity needs.

Job Overview:

This full-time position offers a competitive salary of $26.00 - $27.00 per hour (based on education) and a range of benefits, including medical, dental, and vision insurance, as well as opportunities for professional development and advancement.

Key Responsibilities:
  • Coordinate and participate in team settings to deliver family-driven, youth-guided services
  • Build trusting relationships with families to ensure they have the support needed to be successful
  • Provide health home care coordination services for children with low to high acuity needs
  • Document all case activity, including persistent outreach, consent development, conducting assessments, crisis supports, plan of care development, client progress, and transition arrangements
  • Inventory and coordinate existing services relevant to the Plan of Care, identifying and securing additional services as appropriate

The ideal candidate will have a Bachelor's degree with a minimum of two years experience or a Master's degree with a minimum of one year experience, as well as relevant experience in serving children and families in child welfare, developmental disabilities, behavioral health, primary health care, or social services.
