Job Summary:

This position is responsible for the coordination and delivery of clinical behavioral health services designed to meet the wellness and recovery needs of the consumer, including services provided to victims of violence.

Key Responsibilities:
  • Intake: Conduct intake interviews, prepare diagnostic histories, and collect, analyze, and record significant social and personal factors.
  • Clinical Services: Provide individual, group, and family counseling, as well as supportive counseling, including psych-education groups for those directly impacted by gun violence and their families.
  • Documentation: Maintain clinical documentation in the agency's electronic health record (EHR), including assessments, diagnosis forms, treatment plans, progress notes, collateral contacts, group or individual notes, and termination summaries.
  • Case Management: Maintain continuous communication with clients, including outreach for hard-to-engage clients, provide client advocacy services, community-based counseling, and coordinate with referral sources.
  • Productivity: Spend 65% of time in face-to-face consumer contact, provide routine productivity reports to supervisors, and submit schedules, statistical reports/logs, and clinical reports within prescribed timelines.
  • Supervision: Possibility of supervising graduate students fulfilling field work requirements toward a Master's Degree in Social Work.
Requirements:
  • Master's Degree in Social Work, Counseling, or other appropriate program from an accredited graduate school.
  • Clinical license required (LCSW, LPC, LAC, LSW).
