Key Responsibilities:- Planning: Develop and implement individualized plans of care for patients, taking into account their unique needs and goals.
- Assessment and Evaluation: Conduct comprehensive assessments of patients' health status, environment, and support systems, and continually evaluate their progress and response to care.
- Service Delivery: Provide specialized nursing skills, including wound care, medication management, and infusion therapy, in accordance with patients' plans of care and provider orders.
- Coordination of Services: Communicate effectively with patients, caregivers, providers, and interdisciplinary team members to ensure seamless care coordination and implementation of plans of care.
- Documentation and Record-Keeping: Maintain accurate and complete clinical records, including documentation of patient interactions, assessments, and care provided.
- Current, unrestricted registered nursing license with the Minnesota Board of Nursing.
- BLS Certification or ability to obtain within two months.
- Reliable transportation and current car insurance.
- Ability to care for adult, geriatric, and pediatric patients in an independent fashion in the home setting.
- Home Care experience preferred, with knowledge of reimbursement, prior authorization, OASIS, and Medicare rules and regulations.
- Bachelor's degree in nursing.
- Previous Home Care experience.
- Public Health Nurse certification.
