We are seeking an experienced RN to lead our care management team at Providence Medford Medical Center. As a Clinical Supervisor, you will be responsible for the daily coordination and management of case managers, coordinators, and associates. Your expertise in case management and strong leadership skills will ensure timely provision of coordinated patient care. About the Role:
As a Clinical Supervisor, you will oversee the day-to-day activities of the care management team, ensuring that all staff members have the necessary resources and support to provide high-quality patient care. You will also develop and implement operational plans that align with the vision and goals of the department. With a strong focus on quality improvement, you will work closely with other departments to ensure seamless transitions and optimal patient outcomes. Requirements:
To be successful in this role, you will need:
- A Bachelor's Degree in Nursing or equivalent experience
- At least 3 years of experience in discharge planning and case management in an acute care setting
- Working knowledge of MS Office Suite, including Word, Outlook, and Excel
