We are seeking board-certified nurse practitioners to perform in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. As a contractor with Signify Health, you will have the opportunity to earn up to $2,500 to $4,000 each week by conducting in-home visits focusing on overall health and well-being, identifying potential barriers to care, and earning additional compensation through our pay-per-test model.

About the Role:

As a board-certified nurse practitioner contractor, you will be responsible for conducting in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. You will be provided with a pre-populated, patient-specific form for each evaluation, and will not prescribe medicine, order lab tests, or alter people's current treatment regimen.

Requirements:

  • Board certification as a nurse practitioner
  • Current licensure as a nurse practitioner
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
