Job Description:
Responsibilities
- Care Coordination: Develop and manage individualized care plans for hospice patients, ensuring all aspects of care (medical, psychological, social) are addressed and aligned with the patient's needs and wishes.
- Patient Assessments: Conduct regular assessments of patients' medical, emotional, and social needs, and update care plans as necessary.
- Collaboration: Work closely with physicians, nurses, social workers, chaplains, and other members of the interdisciplinary team to ensure comprehensive care delivery.
- Advocacy: Act as a patient advocate, helping families navigate complex healthcare decisions and ensuring the patient's voice is heard in the care process.
- Education and Support: Provide education to patients and families on hospice services, symptom management, and coping strategies. Offer emotional and psychological support to both patients and their families.
- Documentation: Maintain accurate and timely case management records, including assessments, care plans, and progress notes, in compliance with regulatory guidelines.
- Compliance: Ensure that all care services meet industry standards and regulatory requirements for hospice care.
- Discharge Planning: Assist with transition plans for patients as necessary, including discharge or transfer to another care facility when appropriate.
- A valid nursing license and relevant experience in hospice care.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to assess patient needs and develop individualized care plans.
- Knowledge of hospice care regulations and standards.
- Familiarity with electronic medical records and documentation software.
