We are seeking a highly skilled and compassionate Intake Coordinator to join our team at Turning Point Community Programs. As an Intake Coordinator, you will play a critical role in providing psychosocial, rehabilitation, and recovery-oriented services to our members.
Key Responsibilities- Develop and implement person-centered treatment plans that address the unique needs and goals of each client.
- Conduct interviews with clients and their families to assess their needs and develop a plan for admission to our program.
- Coordinate with referral sources, including hospitals and outpatient providers, to ensure a smooth transition for clients.
- Provide on-the-spot counseling and support to clients in crisis, as well as participate in in-service training and workshops to enhance your skills and knowledge.
- Assess clients for involuntary hospitalization and complete necessary forms, as required.
- Ensure the safety, health, and well-being of our members, and complete paperwork in a timely and efficient manner.
- A master's degree in Social Work or a related field, and valid registration with the Board of Behavioral Sciences (BBS).
- A California driver's license and current vehicle insurance/registration, if driving.
- Reliable means of transportation capable of passing a vehicle safety inspection.
