LPN Care Coordinator

MAB Community Services is seeking a skilled and compassionate LPN to join our team as a Care Coordinator. In this role, you will provide high-quality healthcare services to individuals with acquired brain injuries in residential settings.
Key Responsibilities- Provide wound care, medication administration, and g tube care to participants
- Develop and implement healthcare standards, policies, and procedures
- Review incident reports and conduct audits to ensure program compliance
- Conduct in-service training to Adult Disability Service employees
- Identify and facilitate the utilization of community resources
- Provide client-specific consultation to program staff and healthcare providers
- Current LPN license
- Valid US driver's license for at least one year
- Ability to pass a CORI, reference, multi-state background, and driving record check
