MAB Community Services is a non-profit organization dedicated to providing support services to individuals with developmental disabilities, acquired brain injuries, and other challenges. We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in Shrewsbury, MA.

Job Summary

The LPN will work in a community-based program serving individuals with acquired brain injury, behavioral, and medical needs. The successful candidate will provide high-quality patient care, develop and implement health care standards, and collaborate with the healthcare team to ensure the best possible outcomes for our clients.

Responsibilities
  • Assist with the development and implementation of health care standards, policies, and procedures.
  • Provide training and consultation to Residential Supports Managers, Residential Coordinators, and direct care staff regarding policies and procedures according to Medication Administration Program (MAP) policy guidelines.
  • Review incident reports involving health and safety issues, including mental health and behavioral incidents.
  • Oversee the MAP Program at the residence by conducting routine audits of documentation, providing specific and systemic response to medication errors, and providing on-call consultation on a rotating basis.
  • Conduct reviews of health care documentation to ensure program compliance with provider recommendations, MAB, DDS policies, and MAP policies.
  • Develop and conduct in-service training to Adult Disability Service employees.
  • Identify and facilitate the utilization of appropriate community resources.
  • Provide client-specific consultation to program staff, in conjunction with healthcare providers.
  • Transport participants to medical appointments.
Requirements
  • Current Licensed Practical Nurse (LPN) License.
  • Current CPR/First Aid certification.
  • Valid US driver's license for at least one year.
  • Ability to pass a CORI, reference, multi-state background, and driving record check.
  • Covid-19 vaccination or willingness to be vaccinated prior to starting.
