Job Summary

We are seeking a PRN Occupational Therapy Assistant to join our team in Venice, Florida. As an Occupational Therapy Assistant, you will provide skilled occupational therapy services to clients in their homes under the supervision of an Occupational Therapist.

Responsibilities:
  • Provide client care services delegated by the Occupational Therapist
  • Assist in evaluation and development of the rehabilitative plan of care and periodic re-evaluation
  • Participate in the preparation of clinical and daily progress notes
  • Instruct clients and family members in exercise programs and self-care activities
  • Guide and instruct clients in the use of self-help devices and assistive technology
  • Identify and report potential medication issues and adverse reactions
  • Record activities and findings in the clinical record and report to the Occupational Therapist and physician
  • Consult with the Occupational Therapist regarding the outcome of home visits and report physical findings
  • Assist in the evaluation and development of the rehabilitative plan of care and periodic re-evaluation
  • Instruct other health team personnel and family members on occupational therapy techniques
  • Write daily progress notes and submit within 72 hours
  • Prepare periodic summary reports as indicated
  • Attend rehabilitation department staff meetings and case conferences
  • Instruct clients in the care and use of wheelchairs, braces, splints, and prosthetic and orthotic devices
  • Participate in discharge activities and assist the Occupational Therapist in completing the Occupational Therapy Discharge Summary
  • Confirm scheduling of visits with the supervisor/Director of Clinical Services
  • Assume responsibility for self-development through educational programs and individual research
  • Assume other duties deemed appropriate and necessary by the Occupational Therapist
  • Participate in Quality Assessment and Performance Improvement activities as assigned
  • Attend all patient care conferences as scheduled
  • Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, visit notes, and significant change in condition in accordance with agency defined time frames
Qualifications:
  • Graduate of an approved school giving a course of not less than two years for Occupational Therapy Assistants
  • Licensed or registered by the State
  • Criminal background check
  • CPR certification
