Key Responsibilities- Develop and implement comprehensive care plans that integrate patient preferences and values
- Monitor patient progress through regular assessments and evaluations
- Collaborate with interdisciplinary teams to ensure effective implementation of care plans
- Advocate for patients and families to remove barriers and ensure access to necessary resources
- Maintain accurate and timely medical records, ensuring compliance with regulatory requirements
- Registered Nurse licensure from the Michigan Board of Nursing
- BLS Provider certification from the American Heart Association or American Red Cross
- Case Manager certification from the Commission for Case Manager Certification (CCMC) or American Case Management Association (ACMA) preferred
