Atrio Home Care is seeking a compassionate RN Case Manager to join our Grand Rapids team.
Job Summary:We are looking for a skilled and dedicated RN Case Manager to provide direct care to clients in the Grand Rapids area. The ideal candidate will have a strong passion for delivering high-quality care to older adults and possess excellent communication and interpersonal skills.
Responsibilities:- Perform initial and comprehensive assessments to deliver compassionate care to clients
- Collaborate with physicians and other care team members to develop a Plan of Care
- Promote client and family/caregiver independence through counseling and education
- Coordinate client discharge planning
- Identify barriers to desired outcomes and evaluate outcomes of care
- Prepare clinical documentation and progress notes in a timely manner using iPads for the EMR system
- Participate in on-call rotation
- Licensed RN in Michigan; BSN preferred
- Valid Michigan driver's license, proof of auto insurance, and reliable transportation for home visits
- 1-2 years of acute care or skilled nursing experience; prior certified home health experience preferred
