We are seeking a skilled and compassionate Nurse Case Manager to join our team at Trinity Health Michigan. As a key member of our healthcare team, you will be responsible for overseeing and coordinating the discharge planning and utilization management activities for assigned caseloads.
Key Responsibilities:- Facilitate appropriate utilization of resources and efficient patient progression through the continuum of healthcare.
- Develop and implement individualized care plans to ensure the highest quality and most cost-effective care.
- Collaborate with interdisciplinary teams to ensure seamless transitions of care.
- Provide education and support to patients and families to promote self-management and independence.
- Minimum: BSN; or ADN, with BSN to be obtained within 4 years of hire; ADN only, if nationally certified in Case Management.
- Current RN license in the State of Michigan.
- Active BLS Certification.
- Preferred: Certification in Case Management or other related professional certification.
- Two to five years of medical/surgery or chronic disease management experience in an acute care setting.
