Job Title: Health Navigator

Job Summary:

World Relief is seeking a skilled Health Navigator to support newly arrived Afghans in accessing comprehensive healthcare services and building client self-sufficiency in navigating healthcare systems. This role will work with the Resettlement team to provide new arrivals with case management, services, individual coaching, and connection to resources and benefits.

Key Responsibilities:
  • Assess participant needs, strengths, and goals and establish comprehensive action plans with short and long-term goals.
  • Provide comprehensive, client-centered health navigation services to newly arrived Afghans, including enrolling and supporting in accessing all eligible insurance and healthcare benefits, ensuring clients establish primary care and have access to all needed specialty services, and assisting with booking healthcare appointments and scheduling transportation and interpretation through insurance.
  • Identify appropriate service providers to meet client health and wellness needs, successfully coordinate client referrals, and assist and advocate for ongoing access to services and care.
  • Provide individual coaching to promote self-sufficiency in independently navigating the healthcare system.
  • Work with cross-departmental staff to ensure client integration and referrals across programming.
  • Support and build ongoing partnerships with local service providers and community organizations addressing healthcare needs.
  • Document participant services and activities thoroughly and in a timely manner in accordance with program and World Relief Modesto requirements.
  • Maintain accurate, timely, and compliant casefiles.
  • Update and maintain client information in protected, online databases.
  • Conduct regular, re-occurring meetings with clients to ensure progress on goals and to provide appropriate connections to resources, including transportation assistance.
  • Achieve grant requirements and deliverables through timely, high-quality services.
  • Participate in on-going training to improve service delivery and ensure compliance.
  • Review and regularly present the status and progress of each case to Program Manager.
  • Collaborate successfully across World Relief Modesto staff and departments, as well as with external service providers to ensure holistic case management.
  • Attend meetings as instructed by supervisor.
  • Other duties as assigned.
Requirements:
  • Mature and personal Christian faith.
  • Committed to the mission, vision, and values of World Relief.
  • Desire to serve and empower the Church to impact vulnerable communities.
  • Able to affirm and/or acknowledge World Relief's Core Beliefs, Statement of Faith, Christian Identity, and National Evangelicals For the Health of The Nation document.
  • Bachelor's degree in social work or related field of study; or equivalent work experience.
  • Cross-cultural experience.
  • Strong communication skills, with fluency in written and spoken English; bilingual in Dari/Farsi required.
  • Local travel and transportation of program participants required- valid license, access to reliable vehicle and clean driving record.
