As a Private Duty Nurse LPN, you will play a vital role in our mission, working collaboratively with patients, families, and healthcare professionals to achieve optimal outcomes. This engaging and challenging role offers a salary range of $48,000 - $58,000 per year, depending on experience, plus a rich benefits package.
Key Responsibilities:
- Develop and implement individualized care plans tailored to patient needs and goals
- Conduct thorough patient assessments and update care plans accordingly
- Collaborate with multidisciplinary teams to ensure seamless care coordination and communication
- Educate patients and their families on health promotion, disease prevention, and management strategies
- Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements:
- Active, unencumbered RN/LPN/LVN license in the state of practice
- Current CPR certification from American Red Cross or American Heart Association
- TB skin test (current within last 12 months)
- At least six months of prior hands-on nursing experience preferred but not required
- Excellent communication and interpersonal skills
