Role Summary:Under the supervision of the team leader, the role involves delivering case management and support coordination services within home, office, and community settings for children and adolescents facing serious emotional disturbances (SED), developmental disabilities, or co-occurring disorders. Responsibilities include:
- Developing individualized, family-centered plans in collaboration with children, families, and their support networks.
- Conducting assessments, linking families to resources, and monitoring service delivery.
- Assisting families in navigating educational systems, legal matters, child welfare, and community resources.
- Working alongside families, professionals, and natural support systems to foster a supportive environment for children.
- Performing related duties as necessary.
Qualifications:
Educational Background:
- Bachelor's degree in Social Work or a related human services field (such as Education, Counseling/Guidance, or Psychology).
Experience:
- Minimum of one (1) year of experience in evaluating and treating children or adolescents.
Additional Requirements:
- Possession of a valid Michigan driver's license and current automobile insurance, along with access to a vehicle during work hours.
- Ability to perform the physical tasks associated with the role.
Preferred Qualifications:
- Candidates with a Michigan Bachelor Level Social Worker (LBSW) credential are preferred.
