Occupational Therapist Job Summary

As an Occupational Therapist with Common Sail Investment Group, you will provide essential services to home care clients in accordance with their Plan of Care. Under the direction of the attending physician, you will work with the family and other members of the interdisciplinary team to deliver high-quality care.

Key Responsibilities
  • Perform Occupational Therapy assessments, diagnostic tests, and skilled procedures for clients receiving services under a medically approved Plan of Care.
  • Complete initial assessments within 48 hours of referral, unless other arrangements are made.
  • Document all findings, plans, interventions, and outcomes, and submit documentation to the Agency within 7 days or per Agency policy.
  • Communicate plans and changes to the physician and to the client Case Manager and other caregivers through the care plan, progress notes, and participation in care conferences.
  • Implement teaching appropriate to the client's needs and within the context of their environment and learning ability.
  • Participate in discharge planning and document this in the client record.

Requirements
  • Graduate of an accredited Occupational Therapy curriculum, accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association, or eligible for the National Registration Examination of the American Occupational Therapy Association.
  • Possesses current registration or license as required by state(s) of practice.
  • Has a minimum of two (2) years' experience in a healthcare facility or equivalent experience.
  • CPR certification.
  • Licensed driver with automobile insured in accordance with state/Agency requirements. Reliable transportation with a good driving record.
