Job Summary:

Sapphire Nursing at Meadow Hill is seeking a skilled LPN (Licensed Practical Nurse) to join our NIGHT (11PM-7AM) shift team. As a key member of our healthcare team, you will provide direct nursing care to residents and attend to their personal needs with compassion and dedication.

Responsibilities:

  • Deliver high-quality nursing care in structured and routine situations.
  • Administer and document medications and treatments as assigned by our medical team.
  • Assist with diagnostic procedures and treatments, ensuring the highest level of patient care.
  • Provide health care instructions to patients and families, promoting a smooth transition to home care.
  • Participate in planning, implementing, and evaluating nursing care, ensuring the best possible outcomes for our residents.
  • Assist in patient admission, transfer, and discharge, maintaining accurate records and ensuring seamless transitions.
  • Observe and report patients' vital signs and general condition, providing timely updates to our medical team.
  • Maintain patient charts and environmental conditions, adhering to our facility's high standards of cleanliness and organization.
  • Assist with unit management and direct nurse's aides, promoting a positive and supportive work environment.
  • Maintain clinical competence through education programs, staying up-to-date on the latest medical advancements and best practices.

Requirements:

  • LPN NY License.
  • Prior experience in skilled nursing is a plus, but not required.
