About the Role

Sapphire Nursing at Meadow Hill is seeking a dedicated and compassionate LPN (Licensed Practical Nurse) to join our team on the night shift. As a valued member of our healthcare team, you will provide high-quality patient care and support to our residents.

Responsibilities
  • Deliver nursing care in structured and routine situations.
  • Administer and record medications and treatments as assigned.
  • Prepare for and assist nurses and others with diagnostic procedures and treatments.
  • Reinforce, support, and provide health care instructions to patients and their families.
  • Participate with other team members in planning, implementing, and evaluating nursing care and other patient activities.
  • Assist in the admission, transfer, and discharge of patients.
  • Observe and report patients' vital signs, symptoms, and general condition to the appropriate individuals.
  • Record on patient charts observations of patients' responses to nursing care rendered.
  • Maintain environmental conditions conducive to the comfort and safety of patients.
  • Assist with unit management and/or assume charge of the unit as assigned.
  • Perform other nursing functions related to patient care and nursing service, as delegated by the Nursing Service Administration.
  • Guide and direct other technicians and nurse's aides in rendering patient care.
  • Maintain clinical competence through participation and attendance at continuing education programs.
Requirements
  • LPN NY License
  • Prior skilled nursing experience is a plus
