Job Opportunity

We are seeking a compassionate and experienced MSW to join our Hospice team at AdvisaCare Home Health and Hospice. As a Hospice Social Worker, you will play a vital role in enhancing the lives of patients with life-limiting illnesses and their families.

Key Responsibilities:

  • Conduct initial assessments and develop goal-directed care plans to address the psychosocial, emotional, and spiritual needs of patients and families.
  • Provide care under the direction of a physician and participate in interdisciplinary team conferences to ensure comprehensive care.
  • Document patient and family interactions, referrals, and other relevant information in a timely and accurate manner.
  • Offer crisis intervention, individual or family counseling, and bereavement support as needed.
  • Collaborate with community agencies and services to promote interagency cooperation and facilitate referrals.
  • Participate in the Bereavement Team and adhere to all Hospice policies.

Requirements:

  • MSW degree from a Council on Social Work Education (CSWE) accredited school of social work.
  • One year of experience in Home Health Care, with Hospice experience preferred.
  • Valid Driver's License, car insurance, and reliable transportation.
  • Required Michigan Master's Degree of Social Work.
