Job Summary

Axess Family Services is seeking a skilled Behavioral Health Specialist to join our team. In this role, you will provide psychosocial assessment, consultation, and brief intervention for psychological/psychiatric problems and/or disorders, including chemical dependency.

Key Responsibilities:

  • Develop and implement effective treatment plans to assist clients in achieving their goals.
  • Collaborate with patients and their families to adjust to episodes of acute and chronic illness and promote positive outcomes.
  • Evaluate crisis situations and apply appropriate interventions to ensure the well-being of clients.
  • Participate in meetings that support the Integrated Health Care model to provide comprehensive care for clients.
  • Identify and develop plans to prevent further psychological or physical deterioration in at-risk patients.
  • Assist the primary care team in developing care management processes to improve self-management of chronic disease.
  • Provide comprehensive counseling to patients enrolled in medication-assisted treatment (MAT) programs.
  • Monitor the site's behavioral health program and make recommendations for improvement.
  • Facilitate outreach and involvement with community-based programs and agencies.
  • Provide consultation to other healthcare professionals as needed.

Requirements:

  • Master's Degree in Behavioral Health, Social Work, Counseling, or a related field.
  • Working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.
  • Ability to work effectively in a fast-paced primary care environment and communicate with an interdisciplinary team.
  • Good knowledge of psycho-pharmacology and the ability to design and implement clinical pathways and protocols for treatment of selected chronic conditions.
  • Multilingual, multicultural, and culturally diverse applicants are encouraged.

Licenses and Certifications:

  • Licensed in the State of Ohio as a Licensed Independent Social Worker (LISW)/Licensed Social Worker (LSW) required. Or
  • Independent Marriage and Family Therapist Or
  • Licensed Professional Clinical Counselor (LPCC)/Licensed Professional Counselor (LPC)
