Job Title: Home Health Physical Therapy Assistant

Alternate Solutions Health Network LLC is seeking a skilled Home Health Physical Therapy Assistant to join our team in Bellevue, Kentucky.

Job Summary:

We are looking for a compassionate and detail-oriented Physical Therapist Assistant to provide high-quality care to our patients in their homes. As a Home Health Physical Therapy Assistant, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that promote patient independence and well-being.

Responsibilities:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Assess patient conditions and notify the Physical Therapist of any changes.
  • Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means.
  • Minimize patient risk by assessing functional problems and providing intervention to improve patient functional status.
  • Counsel patients and families on disease/injury management and instruct on proper use of equipment.
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
Requirements:
  • Current Physical Therapist Assistant (PTA) license in the state of employment.
  • Minimum one-year experience as a PTA in an acute care setting, with home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.
