Job Summary

We are seeking a skilled Home Health Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals in their homes, promoting optimal recovery and independence.

Key Responsibilities
  • Plan of Care: Develop and implement individualized plans of care, incorporating physical therapy interventions to address patient needs and goals.
  • Patient Outcomes: Treat patients to relieve pain, restore function, and maintain maximum performance, using evidence-based practices and techniques.
  • Minimize Patient Risk: Identify and mitigate risks, ensuring patient safety and preventing re-hospitalizations through proactive interventions and education.
  • Family Educator/Advocate: Educate patients and their families on disease management, prevention, and self-care strategies, promoting empowerment and independence.
  • Supervision: Oversee and supervise therapy services provided by physical therapist assistants and nurse aides, ensuring high-quality care and adherence to agency policies.
  • Communication/Collaboration: Communicate effectively with interdisciplinary team members, patients, and families, ensuring seamless care coordination and patient-centered outcomes.
  • Policies and Regulations: Maintain accurate clinical documentation, adhering to agency protocols and Medicare/Federal guidelines, while understanding and following agency policies, procedures, rules, and regulations.
Requirements
  • Physical Therapist License: Current licensure in the state of employment.
  • Valid Driver's License: Possess a valid driver's license and auto insurance in your name as a driver.
  • Physical Demands: Capable of meeting all physical demands of the job, including lifting, bending, and standing for extended periods.
