Job Title: Home Health Physical Therapy Assistant

Alternate Solutions Health Network LLC is seeking a skilled Home Health Physical Therapy Assistant to join our team in Bellevue, Kentucky.

Job Summary:

We are looking for a compassionate and detail-oriented Physical Therapist Assistant to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Physical Therapy Assistant, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that promote patient independence and well-being.

Responsibilities:
  • Provide skilled physical therapy services as ordered by the attending physician, including exercises, modalities, and patient education.
  • Assess patient conditions and notify the Physical Therapist of any changes or concerns.
  • Develop and implement plans of care that address patient functional problems and promote independence.
  • Counsel patients and families on disease management and proper use of equipment.
  • Document patient progress and outcomes in accordance with agency protocol and Medicare guidelines.
Requirements:
  • Current Physical Therapist Assistant (PTA) license in the state of employment.
  • Minimum one year of experience as a PTA in an acute care setting, with home care experience preferred.
  • Valid driver's license and auto insurance.
