Job Summary

We're seeking a skilled Advanced Practice Provider (APP) to join our team as a Traveling Advanced Practice Provider. As a key member of our geriatric care team, you'll provide primary, urgent, and acute medical care to patients in their homes, working collaboratively with our physicians, social workers, and care team coordinators to deliver concierge-level care.

Responsibilities
  • Provide primary care to patients in their homes, including diagnosis, treatment, and management of chronic and acute medical conditions
  • Collaborate with our care team to develop and implement individualized care plans that address the unique needs of each patient
  • Facilitate staff, patient, and family decision-making through education and resource provision
  • Address urgent care needs of patients as needed, including palliative and end-of-life care
  • Participate in quality improvement initiatives to ensure high-quality, patient-centered care
Requirements
  • Master's degree in Nursing
  • Valid nurse practitioner license in the state of operations and current board certification in your field
  • 5+ years of progressive care experience, including geriatrics, internal medicine, and palliative care
  • Strong computer skills, including Word, Excel, and PowerPoint
  • Ability to travel up to 100% locally, with a valid driver's license and car insurance
