About the Role

We're seeking a skilled Advanced Practice Provider (APP) to join our team as a Traveling Advanced Practice Provider. As a key member of our care community, you'll work closely with our physicians, social workers, and care team coordinators to deliver concierge-level medical care to our patients.

Responsibilities
  • Provide primary care to patients in their homes, including diagnosis and treatment of chronic and urgent medical conditions
  • Collaborate with our care team to complete care assessments and develop personalized care plans
  • Deliver palliative care and end-of-life care to patients and their families
  • Facilitate family meetings and provide educational resources to patients and their caregivers
  • Participate in quality improvement initiatives to ensure high-quality care and patient satisfaction
Requirements

To be successful in this role, you'll need:

  • A Master's degree in Nursing and a valid nurse practitioner license in the state of operations
  • Current board certification in your field and at least 5 years of progressive care experience
  • A valid driver's license, car insurance, and a willingness to drive to patient homes
  • Strong computer skills, including Word, Excel, and PowerPoint

Travel Requirements

As a Traveling Advanced Practice Provider, you'll be required to travel up to 100% locally to patient homes, hospitals, and other healthcare facilities.
