About the Role

We're seeking a skilled Nurse Practitioner to join our team as a Traveling Advanced Practice Provider. As a member of our geriatric care team, you'll work closely with our physicians, social workers, and care coordinators to deliver concierge-level medical care to seniors in their homes.

Key Responsibilities
  • Provide primary care to patients in their homes, including routine check-ups, chronic disease management, and urgent care services.
  • Collaborate with our care team to develop and implement personalized care plans that meet the unique needs of each patient.
  • Facilitate staff, patient, and family decision-making by providing educational resources and support.
  • Address urgent care needs of patients as needed, including emergency situations and hospital transfers.
  • Deliver quality care while maintaining all company metrics pertaining to documentation, coding, HEDIS measures, patient engagement, and medical expense management.
Requirements
  • Master's degree in Nursing
  • Valid nurse practitioner license in the state of operations and current board certification in your field
  • 5+ years of progressive care experience
  • Valid driver's license, car insurance, and access to a car for travel to patient homes
