Job Summary:
The Providence Center seeks a skilled Residential Case Manager to provide clinical case management services in a residential setting. As a key member of our team, you will work closely with consumers to facilitate program goals and objectives using a quality care approach.
Key Responsibilities:
- Provide case management and support to consumers, promoting independence and self-advocacy.
- Develop and implement individualized treatment plans, incorporating person-centered approaches to address mental health symptoms and co-occurring disorders.
- Collaborate with consumers, family members, and community agencies to ensure comprehensive care and support.
- Provide psycho-social rehabilitation services, including counseling, problem-solving, and skill-building activities.
- Monitor and evaluate consumer progress, making adjustments to treatment plans as needed.
- Participate in interdisciplinary team meetings to share knowledge and best practices.
Requirements:
- Associate's degree in Human Services or a related field; Bachelor's degree preferred.
- Experience working in residential facilities or similar settings.
- Valid driver's license and insured auto.
