RN Case Manager

The Registered Nurse Case Manager provides professional nursing services to patients aged 18 and older with a wide range of diagnoses. This role involves assessing patient needs, developing and implementing care plans, and evaluating their effectiveness. The RN Case Manager is responsible for coordinating nursing care, educating patients and their families, and delegating tasks to staff members. This position requires strong communication and interpersonal skills, as well as the ability to work autonomously and make sound judgments.
Key Responsibilities:
Assess patient conditions and develop care plans in collaboration with physicians and other healthcare professionals.
Coordinate nursing care, including administering medications, providing treatments, and monitoring patient progress.
Educate patients and their families about their care, including medication management, wound care, and other health-related topics.
Delegate tasks to staff members, ensuring that patient care is delivered in a timely and effective manner.
Document patient progress and report any changes or concerns to the clinical team.
Maintain accurate and up-to-date records of patient care, including medication administration, treatment plans, and progress notes.
Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care.
Requirements:
This role requires a Registered Nurse with a strong background in case management and patient care. The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work autonomously and make sound judgments.
Education and Certifications:
Associate's degree in professional nursing (ADN)
Current Basic Life Support (BLS) certification
Valid driver's license and proof of insurance
Work Environment:
This role involves working in a fast-paced healthcare environment, with frequent interactions with patients, families, and healthcare professionals. The RN Case Manager will be required to work autonomously, making sound judgments and decisions in a timely manner.
Physical and Mental Demands:
This role requires the ability to lift up to 50 pounds, stand for prolonged periods, and walk from one location to another. The RN Case Manager will also be required to use their hands and fingers to perform tasks such as typing, writing, and manipulating objects.
Supervision:
The RN Case Manager will receive daily supervision from a Clinical Manager and may provide direction to care givers on a daily basis.
