Overview

Position Summary

Full-Time Role - Standard Business Hours, Monday to Friday.

The ideal candidate should possess direct clinical experience.

ROLE SUMMARY: The Patient Coordinator plays a crucial role in assisting patients as they navigate the healthcare landscape, addressing barriers that may impede their access to necessary care and treatment. This position involves coordinating efforts with various healthcare professionals within the patient Care Team to ensure cohesive collaboration for optimal patient outcomes.

KEY RESPONSIBILITIES: Qualified candidates must demonstrate the capability (with or without reasonable accommodation) to execute the following tasks:
  • Facilitate collaborative meetings with the Care Team to ensure all members are available to discuss shared patients as required.
  • Enhance communication among patients, their families, and healthcare professionals to promote satisfaction and high-quality care.
  • Ensure continuity of care and connect patients with community resources as necessary.
  • Educate patients on the financial aspects of their care and assist them in accessing financial resources when needed.
  • Conduct daily reviews of upcoming provider schedules to gather pertinent information for morning team discussions.
  • Support the Practice Manager in weekly team meetings with in-office providers to review patients receiving care from multiple providers.
  • Act as the primary resource for all inquiries (internal/external) regarding insurance and referrals, possessing comprehensive knowledge of insurance verification and authorization processes.
  • Serve as a liaison between patients and healthcare providers regarding referral processing, utilizing various communication methods.
  • Act as an intermediary between patients and insurance companies to clarify requirements for referral and authorization compliance.
ADDITIONAL RESPONSIBILITIES: The following tasks are considered supplementary to the primary responsibilities outlined above:
  • Maintain familiarity with registration and scheduling procedures, with the ability to assist as needed based on office demands.
  • Perform other duties as assigned.
QUALIFICATIONS

MINIMUM REQUIRED QUALIFICATIONS:
  • High School Diploma or General Equivalency Degree (GED).
  • Three (3) years of experience in clerical, customer service, or administrative support roles within a customer-focused environment.
  • Three (3) years of experience with basic computer skills, including keyboarding and use of office equipment.
  • One (1) year of experience in registration, point of service collection, insurance validation, and understanding of compliance/regulatory guidelines.
  • One (1) year of experience in Revenue Cycle within a healthcare setting.
PREFERRED QUALIFICATIONS:
  • Associates degree or equivalent experience in healthcare is preferred.
  • Prior experience with Electronic Medical Records systems is advantageous.
