Job Title: Registered Nurse - Home Health

ChristianaCare is seeking a skilled Registered Nurse (RN) to join our Home Health team in Deepwater, New Jersey.

Job Summary:

We are looking for a compassionate and dedicated RN to provide high-quality care to our patients in the comfort of their own homes. As a Home Health RN, you will work closely with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient.

Responsibilities:
  • Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
  • Develop and implement care plans that are goal-oriented and patient-centered.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination.
  • Communicate effectively with patients, families, and healthcare providers to ensure optimal care outcomes.
Requirements:
  • BSN required; ADN candidates are required to obtain their BSN within three years of their start date.
  • Current DE RN license or compact state license.
  • 1 year of RN experience.
  • Previous Home Healthcare and Med-Surgical experience preferred.
