Position Overview

The Practical Nursing Specialist (LPN/LVN) plays a crucial role in delivering and documenting proficient nursing care under the guidance of a Registered Nurse. This is executed in alignment with the established care plan and physician directives for each patient, while consistently upholding confidentiality standards and maintaining professional boundaries.

Key Responsibilities
  • Deliver and coordinate high-quality patient care in accordance with physician directives.
  • Continuously monitor and evaluate patient conditions, reporting any changes to the supervising nurse or physician as necessary.
  • Accurately document all activities, assessments, nursing interventions, responses, and care coordination promptly.
  • Engage in the development, implementation, and revision of the nursing care plan.
  • Take appropriate nursing actions based on assessments to achieve desired patient outcomes.
  • Identify changes in patient needs that require intervention and implement care strategies to mitigate risks.
  • Maintain personal and professional accountability by adhering to organizational policies, state and federal regulations, and the Nurse Practice Act.
  • Utilize infection control protocols to safeguard both staff and patients in compliance with OSHA standards.
  • Educate patients and their families about disease processes, self-care techniques, and preventive measures.
  • Stay informed about nursing competencies through participation in educational programs and ongoing training.
Qualifications
  • Graduate from an accredited nursing program.
  • Possess a current, unrestricted state license as a Licensed Nurse in the relevant state.
  • Hold a valid CPR certification.
  • Demonstrated expertise in clinical assessments, documentation, and adherence to nursing policies and procedures.
Preferred Experience
  • At least six months of recent experience as a Licensed Nurse in a clinical setting.
  • Experience in home health care.
Skills and Abilities
  • Proficient in Microsoft Office 360, including Word, Excel, and TEAMS.
  • Ability to maintain confidentiality and professional boundaries at all times.
  • Capable of remaining composed and professional in high-pressure situations.
  • Strong attention to detail and effective time management skills.
  • Commitment to excellence and proficient problem-solving abilities.
  • Exceptional organizational and communication skills.
  • Quick-thinking with astute decision-making capabilities.
Physical Requirements
  • Must be able to communicate effectively in English, both verbally and in writing.
  • Occasional lifting, carrying, pushing, and pulling of up to 25 pounds.
  • Ability to sit, walk, stand, bend, kneel, reach, twist, and climb stairs.
  • Must possess visual and auditory acuity.
Work Environment

This position allows for remote work, requiring availability during designated business hours. The work environment may be demanding due to deadlines and multiple tasks, necessitating compliance with various laws and regulations. Standard office equipment such as laptops and smartphones will be utilized.
