Position Information:
The Care Coordinator is a Registered Nurse who facilitates the plan for inpatient/outpatient care by using a collaborative interdisciplinary approach. The Care Coordinator will focus on anticipation, integration, and coordination of clinical care and discharge planning services necessary to meet the needs of patients and to optimize clinical and resource outcomes.

Responsibilities:
Collaborate with the interdisciplinary team in assessing patient needs and developing the plan of care.
Act as an advocate for the patient, physician, and hospital relative to compliance with federal, state, and third-party utilization management regulatory requirements.

Requirements:
3 years of experience in a nursing-related area.
Clinical experience in case management, utilization management, or home or community health is preferred.
Graduate degree in nursing is required, and a Bachelor's degree in nursing is preferred.
Registered Nurse licensure is required.

Work Schedule:
Monday, Tuesday, and Wednesday, 2pm to 10pm.

Weekend and Call Requirements:
No weekends or call requirements.
