A Home Health Aide plays a vital role in delivering personalized care to patients in the comfort of their own homes. As a member of our team, you will provide direct patient care under the guidance of a registered nurse and in accordance with the Aide Plan of Care (POC).
- Assist patients with self-administered medications, ensuring their safety and well-being.
- Take accurate temperature, pulse, and blood pressure measurements, recognizing and reporting any abnormal results to the supervisor.
- Help patients maintain good personal hygiene by performing or supervising bathing, grooming, skin care, and other essential activities.
- Assist with feeding patients, promoting healthy nutrition and hydration practices.
- Support patients with mobility, exercises, positioning, and transfers, as outlined in the Plan of Care and CenterWell Home Health policy.
- Document interactions with patients, caregivers, doctors, and other staff members, ensuring continuity of care and accurate record-keeping.
- Practice infection control principles, maintaining a clean and safe environment for patients.
- Perform other household duties, such as light laundry, bed changing, and meal preparation, as needed.
To be successful in this role, you will need:
- A high school diploma or equivalent.
- Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months.
- Meet applicable state certification requirements.
- A valid driver's license, auto insurance, and reliable transportation.
- Good standing on the HHA Registry (if applicable) and completion of HHA/CNA course to work for a Medicare-certified agency.
- At least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency.
Compensation and Benefits
CenterWell Home Health offers a competitive compensation package, including a salary range of $31,200 - $40,600 per year, depending on experience and qualifications.
