Role Overview
The RN Care Manager is responsible for orchestrating and prioritizing care for individuals and groups of patients, with an emphasis on strategies that enhance overall health within populations. This role requires a deep understanding of nursing care and the management of a varied patient caseload, aiming to elevate outcomes for patients, families, and the broader health system through educational initiatives and preventive care within a managed care framework.
- Evaluates both internal and external referrals to ascertain the needs of patients and their significant others, including the intensity of care required, insurance coverage, and available resources.
- Creates comprehensive care plans and advises primary care providers, specialists, and other healthcare team members on effective care management strategies, ensuring continuity of care throughout the healthcare continuum.
- Identifies the educational requirements of patients, families, and healthcare team members, developing and executing suitable teaching methods or making necessary referrals.
- Facilitates communication and collaboration among patients, their significant others, providers, and payers to streamline access to appropriate services, promoting optimal health in a cost-efficient manner.
- Evaluates patient care outcomes, interprets data reports, and analyzes trends for patient groups.
- Records patient information, care plans, interventions, and outcomes in accordance with departmental standards.

Qualifications
- Bachelor's Degree in Nursing is required.
- Must possess a Registered Nurse license in the State of Michigan.
- A minimum of 5 years of relevant clinical experience is preferred.
- At least 2 years of experience in clinical nursing or case management/managed care is required.
