RN Case Manager

The University of California, Irvine is seeking a skilled RN Case Manager to join our healthcare team. As a key member of our interdisciplinary team, you will play a vital role in ensuring the delivery of high-quality patient care.
Responsibilities- Assess patient treatment needs and develop, monitor, and evaluate treatment plans and progress.
- Facilitate interdisciplinary approaches to patient care, coordinating and providing care that is safe, timely, effective, efficient, and patient-centered.
- Collaborate with healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and care coordination.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Current California RN license.
- Three to five years of experience in an acute care hospital setting (medical/surgical preferred).
- Previous one to two years of experience as a Case Manager.
- Proficiency in typing and computer skills, including Microsoft Office.
- Excellent written and verbal communication skills in English.
- Broad knowledge of Medicare, Medi-Cal, and insurance guidelines.
- Critical care experience (for Resource Case Manager).
