Job Title: Travel Nurse RN Case Manager
Job Summary: Healthcare Support is seeking a skilled Travel Nurse RN Case Manager to join our team.
Job Responsibilities:
- Provide high-quality patient care in an acute care setting, utilizing strong case management skills to ensure optimal patient outcomes.
- Collaborate with healthcare teams to develop and implement effective care plans, promoting patient safety and satisfaction.
- Conduct thorough assessments of patient needs, identifying opportunities for improvement and implementing evidence-based interventions.
- Develop and maintain strong relationships with patients, families, and healthcare providers, fostering a culture of open communication and trust.
- Stay up-to-date with industry developments and best practices, applying knowledge to improve patient care and outcomes.
Requirements:
- Active RN license in good standing.
- 2+ years of acute hospital case management experience, with a strong focus on patient-centered care.
- Utilization review experience highly preferred.
