RN Case Manager

The University of California, Irvine is seeking a skilled RN Case Manager to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality patient care.
Key Responsibilities- Assess patient treatment needs and develop, monitor, and evaluate treatment plans and progress.
- Facilitate interdisciplinary approaches to patient care, coordinating and providing care that is safe, timely, effective, efficient, and patient-centered.
- Collaborate with healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and care coordination.
- Current California RN license.
- Three to five years of experience in an acute care hospital setting, with a focus on medical/surgical care.
- Previous experience as a Case Manager, with a strong understanding of Medicare, Medi-Cal, and insurance guidelines.
- Excellent written and verbal communication skills, with the ability to maintain effective working relationships across the Health System.
- Broad knowledge of University and medical center organizations, policies, procedures, and forms.
- Critical care experience, with a focus on resource management and patient care coordination.
