Job Summary

Under the supervision of a licensed nurse, performs a variety of non-professional patient care duties for Kaiser Permanente members in a skilled nursing facility. The ideal candidate will be responsible and accountable for assigned tasks, working collaboratively with the healthcare team to provide high-quality patient care.

Key Responsibilities
  • Assists with Patient Care Planning: Provides input to the registered nurse for the development and revision of patient care plans, ensuring that individualized care is provided to meet the unique needs of each patient.
  • Collects and Reports Patient Data: Assists with ongoing data collection, identifying changes in patient status and reporting these changes to the registered nurse or licensed practical nurse.
  • Implements Care Plans: Sets priorities of care with licensed staff, ensuring that patient needs are met in a timely and efficient manner.
  • Supports Admissions, Discharges, and Transfers: Assists with admissions, discharges, and transfers, ensuring a smooth transition for patients and their families.
  • Reports Incidents and Concerns: Reports incidents, work-related injuries, and unsafe equipment or environments to the supervisor or other appropriate personnel.
  • Practices Infection Control: Functions with awareness of and applies infection control principles and standards, including patient, environment, and employee issues.
  • Participates in Quality Improvement: Participates in assigned quality improvement activities, contributing to the continuous improvement of patient care and services.
  • Communicates Effectively: Reports pertinent patient data at shift report, reports problems to licensed staff, charge nurse, or supervisor, and practices effective communication with patients, families, and healthcare team members.
Requirements
  • Education: High school diploma or equivalent required; hospital aide training program or documented completion of equivalent military training course preferred.
  • Certifications: Nurse aide certification (Hawaii) within 3 months of hire; basic life support certification from the American Heart Association within 1 month of hire.
  • Experience: Minimum one year of experience as a hospital aide preferred.
