LPN Medication Care Manager

At Sunrise Senior Living, we are seeking a highly skilled and compassionate LPN Medication Care Manager to join our team. As a Medication Care Manager, you will play a vital role in ensuring the highest quality of care for our residents by administering medications and treatments in a safe and organized manner.
Responsibilities:- Administer medications and treatments to residents in accordance with their individualized care plans.
- Conduct regular medication reviews to ensure accuracy and safety.
- Document all medication administration and resident interactions in the resident's medical record.
- Collaborate with the Resident Care Director and other healthcare professionals to ensure seamless care coordination.
- Participate in ongoing education and training to stay up-to-date on the latest medication management practices and regulations.
- Current LPN licensure in the state of employment.
- Minimum of 1 year of experience in a long-term care setting.
- Strong knowledge of medication management principles and practices.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
