Job Summary

Sunrise Senior Living is seeking a highly skilled LPN Medication Care Manager to join our team. As a Medication Care Manager, you will play a vital role in ensuring the highest quality of care for our residents. This is a full-time overnight position available.

Key Responsibilities
  • Medication Administration: Administer medications and treatments in a safe and organized manner, ensuring compliance with regulatory requirements and Sunrise Senior Living policies.
  • Documentation: Accurately document medication administration, including reading, notating, and initializing the Daily Log.
  • Communication: Receive medication updates from the Resident Care Director (RCD) or Wellness Nurse and communicate effectively with the care team.
  • Medication Management: Ensure medications are passed according to schedule, document refusals or missed doses, and maintain confidentiality of resident information.
  • Risk Management: Identify and report any resident concerns, accidents, or incidents, and participate in risk management programs and policies.
Resident Care
  • Notifying RCD: Notify the RCD of any resident and/or family concerns through the LEAD process.
  • Participating in Cross Over Meetings: Attend and actively participate in daily Cross Over meetings facilitated by the Lead Care Manager (LCM).
  • Conducting Service and Health Updates: Conduct Service and Health Updates as directed by the RCD.
  • Individualized Service Plans (ISP)/Individualized Care Plans (ICP): Participate in the development of ISP/ICP and monthly updates.
Training and Contributing to Team Success
  • Participating in Training: Attend regular training sessions by the RCD and neighborhood coordinators.
  • Team Collaboration: Actively participate as a member of a team and commit to working toward team goals.
  • Customer and Team Member Engagement: Contribute to the overall engagement programs and processes, including participating in the team member engagement survey and engagement improvement planning workshops.
Qualifications
  • LPN License: Must be a licensed practical nurse (LPN) with a valid license in the state/province where the community is located.
  • Certifications: Maintain certifications in CPR and First Aid, as well as any other certifications required by Sunrise Senior Living and state/provincial regulations.
  • Experience: Previous experience working with seniors is preferred.
  • Skills: Possess excellent written and verbal communication skills, good judgment, problem-solving, and decision-making skills.
