Job Summary

Clinity Talent is seeking a highly skilled Psychotherapist to join our team in a community health setting. As a key member of our primary care team, you will work closely with clients to provide comprehensive behavioral health services.

Key Responsibilities
  • Implement the Screening, Brief Intervention and Referral to Treatment (SBIRT) program, administering standardized screening tools to identify clients with mental health and substance abuse issues.
  • Provide brief treatment or intervention to clients with substance use issues, where appropriate.
  • Coordinate behavioral health care and/or substance use treatment as needed, based on screening, brief intervention, and assessment of client service needs.
  • Refer and coordinate care for clients with mental health and/or substance use problems to an appropriate specialty provider.
  • Contribute to clients' treatment planning, participate in team huddles, and monitor and communicate clients' needs and progress to both primary medical providers and behavioral healthcare providers.
  • Communicate and collaborate with clients' care managers to support long-term relationships in behavioral health treatment.
  • Facilitate communication between the primary care team and specialty behavioral health providers, ensuring all team members are up-to-date on clients' behavioral health status, including diagnoses, medication lists, treatment compliance, and social functioning.
Requirements
  • Master's degree in Social Work (MSW), Licensed Master's Social Worker (LMSW), Master's in Human Counseling (MHC), or Licensed Mental Health Counselor (LMHC).
  • At least 1 year of post-graduate experience working with individuals or families with a history of substance use, mental illness, chronic homelessness, and/or HIV/AIDS.
  • Bilingual Spanish is strongly desired.
