Job Title: Dialysis Nurse - Chronic Care

We are seeking a skilled Dialysis Nurse to join our team at The Grand Rehabilitation and Nursing in Pawling, NY. As a Dialysis Nurse, you will be responsible for providing high-quality care to our geriatric patients with chronic kidney disease.

Responsibilities:
  • Administer dialysis treatments to patients with chronic kidney disease
  • Monitor patients' vital signs and adjust treatment as needed
  • Collaborate with healthcare team to develop and implement individualized care plans
  • Provide education to patients and families on dialysis treatment and self-care
Requirements:
  • 1 year or more of current dialysis experience
  • Geriatric patient experience
  • NXStage Dialysis Machine experience (strongly preferred)
  • CNN or CDN Certification (preferred)
  • Crown Web EMR experience (preferred)
  • BSN (preferred)
