Overview:
This role is responsible for coordinating and delivering comprehensive medication management to enhance the effectiveness of medication therapy.
Key Responsibilities:
- Analyze, interpret, and assess medication regimens to identify, prevent, and resolve medication-related issues.
- Deliver patient care by initiating, adjusting, and monitoring medication therapies through approved methods, including therapeutic interchange and collaborative practice agreements.
- Provide drug information and educational resources to healthcare professionals, students, and patients.
- Collaborate with the healthcare team to develop, implement, and assess quality improvement initiatives in medication management.
- Demonstrate critical thinking skills, sound judgment, and the ability to operate with minimal supervision in a fast-paced environment.
Qualifications:
- Bachelor’s Degree in Pharmacy from an accredited institution.
- Valid state licensure as a Pharmacist and any necessary life support certifications as required for the position.
